Results of Isolated Cementless Acetabular Revision

Abstract

|
Purpose: The clinical and radiologic results of isolated acetabular revision using a cementless acetabular component without removal of a well-fixed femoral component were reviewed.
Materials and Methods: Thirty-one, isolated, cementless acetabular revision cases (12 men, 19 women) that were followed up longer than 5 years were included in the study. All femoral components that had been in place for a mean of 7.7 years were well-fixed at the index surgery. During revision, the acetabular defect was filled with morselized cancellous bone graft and a cementless acetabular cup was inserted. In two hips, additional structural autograft was used because of significant wall defect.
Results: Average Harris hip score was improved from preoperative 53 points to postoperative 87 points, and 26(85%) hips showed a good or excellent clinical results. Two(6%) acetabular components failed and both were revised. The other 29(94%) femoral components remained well-fixed at the last follow-up. One cemented femoral component was waiting for revision because of definite loosening and another cementless femoral component had been revised for osteolysis.
Conclusion: Isolated acetabular revision can be recommended for isolated acetabular aseptic loosening without revising a well-fixed femoral stem as it does not compromise the final outcome and avoids serious potential complications.
